Ok heres a go a the 1up mushrooms

Duration- 10 minutes/level
When eaten these magical mushrooms function like the raise dead spell , except that you are able to restore life and complete strength to any deceased creature.
When a player under the effects of a 1Up mushroom dies; he automatically comes back to life in 1d6+1 rounds. The creature is immediately restored to full hit points, vigor, and health, with no loss of prepared spells. However, the subject loses one level or 2 points of Constitution if the subject was 1st level. (If this reduction would bring its Con to 0 or lower, it can’t be resurrected). This level loss or Constitution loss cannot be repaired by any means. Also any other magical effects cast upon the subject are lost (excepting those granted by objects worn by the subject). The effect created by the 1up mushroom cannot be dispelled. When the subject steps into an anti-magic field the effect is negated until the user steps out of the field. If a player dies while in such a field he will not automatically return back to life until his body is removed from the field. Assuming the mushroom’s effect is still in affect, the subject will return in the usual 1d6+1 rounds. Once the mushroom resurrects a creature, the mushroom's effects are ended and a creature must eat another one to regain this effect.
The condition of the remains is not a factor. So long as some small portion of the creature’s body still exists, it will be resurrected, but the portion receiving the spell must have been part of the creature’s body at the time of death. (The remains of a creature hit by a disintegrate spell count as a small portion of its body.)
The 1Up mushroom can resurrect someone killed by a death effect or someone who has been turned into an undead creature and then destroyed. You cannot resurrect someone who has died of old age. Constructs, elementals, outsiders, and undead creatures can’t be resurrected.
